Welcome package for new FST clients
If you are interested in accessing counselling services at Family Service Toronto, please contact our Service Access Unit at 416 595 9618.
Our intake staff will talk with you about your situation and help you determine what type of counselling you would benefit from. They will ask you to provide some basic information about yourself. They will also determine your availability and which location you prefer to receive service at. Following this you will be registered for service in the program that best fits your needs.
When a time comes available for you to meet with a counsellor, you will be contacted by phone and offered an appointment. Upon confirmation of your appointment, a confirmation form will be sent to you via secure email. Secure email is a form of registered electronic mail. Service Access staff will explain how to use this system. This document will contain details of your appointment time, counsellor’s name and contact information as well as links to documents which you will be asked to review at your first appointment.

- Clients rights and responsibilities
- Client information form (when you arrive at FST, you will be given this form with the information you provided to us when you first contacted FST. You will be asked to confirm this information and fill in any missing data)
- Acknowledgement form
- A letter from FST’s Executive Director which introduces how FST works with clients to determine if they are benefitting from their work with us.
